Before you start
- Have your identity or work-right documents ready.
- Have your Tax File Number, bank and super fund details ready.
- Choose a trusted emergency contact.
- Allow about 25–35 minutes.
Accessibility and language
The form supports keyboard navigation, browser zoom, screen readers, large touch controls and plain English. English and Khmer can be enabled in Portal Settings when approved translations are available. Legal declarations remain visible in English.
Problems uploading a file
Use PDF, JPG, JPEG, PNG or a supported iPhone photo. The maximum is 15 MB. Try a clearer, smaller photo if an upload fails. Your completed answers will remain saved.
Need a correction?
Contact BERRIES NATION using the company support details once configured. For security, do not email TFNs, bank details, medical details or passport copies. An authorised administrator can send a secure, limited correction link.
